The Team The service delivery model of this team focuses on permanency starting at intake and keeps permanency in the forefront of planning through out the child’s journey in or out of care.  Our team services children from 0-15 yrs of age from the point they leave their family home.   We also service and hold the Child and Youth Special Needs (CYSN) children that are in Special Needs agreements from the age of 0-19 yrs and are responsible for managing their transitions over to Community Living BC (CLBC) while maintaining relationships with their parents and including them and the CYSN SW in all planning.  As well, for children in the continuing custody of the director, we also do permanent transfer of custody and adoption planning.    The Role As a Guardian
